The primary role of the Medical Social Worker II is to provide comprehensive social work care to patients and their caregivers in accordance with agency policy and procedure and state and federal regulations. As an integral member of the healthcare team, the Medical Social Worker II delivers age-appropriate social work care to patients and their caregivers, providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of patients and families in the hospital and clinic settings.
Key Responsibilities:- Assessment and Treatment Planning: Conduct psychosocial assessments to identify patient needs and develop individual treatment plans in collaboration with patients and their caregivers.
- Counseling and Support: Provide counseling services to patients and their caregivers, including crisis intervention, grief and bereavement support, and stress reduction techniques.
- Community Resource Referral: Identify and refer patients and their caregivers to community resources and other social work services to meet their complex needs.
- Collaboration and Communication: Work collaboratively with healthcare teams to provide comprehensive care to patients and their caregivers, and communicate effectively with patients, families, and healthcare providers.
- Documentation and Record-Keeping: Maintain accurate and timely documentation of patient interactions, assessments, and treatment plans.
- Education: Master's degree in Social Work from an accredited program.
- Experience: 2-4 years of experience in social work, preferably in a healthcare setting.
- Skills: Strong assessment and treatment planning skills, excellent communication and interpersonal skills, and ability to work effectively in a fast-paced healthcare environment.
- LCSW Certification: Certification as a Licensed Clinical Social Worker (LCSW) is preferred.
- Experience in Healthcare: Experience working in a healthcare setting, preferably in a social work role.
