Job Summary

We are seeking a skilled Clinical Care Manager to join our team at Pacific Clinics. As a key member of our managed care team, you will work closely with Program Managers and Supervisors to ensure high-quality care management assessments and plans in our managed care programs.

Responsibilities
  • Conduct comprehensive care management assessments, including risk and social determinants of health, to develop effective care plans.
  • Apply advanced skills to gather assessment information and ensure quality care.
  • Participate in Quality Improvement activities and project implementation.
  • Perform other related responsibilities as assigned to support departmental needs.
Qualifications

To be successful in this role, you must have a Master's degree in social work or a closely related field, with three years of supervised clinical experience and licensure according to California state law.

Preferred qualifications include a minimum of two years of clinical experience with complex care, experience with care management plans, and knowledge of Social Determinants of Health and Crisis management experience.

We offer a competitive salary range of $66,560.00-$87,000.00, depending on qualifications, skills, experience, and location. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision benefits, retirement options, and more.
