Nurse Case Manager

Mass General Brigham is seeking a skilled Hospice RN Case Manager to join our team.
About the Role:- Provide skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice.
- Communicate and document observations and assessments, promoting and maintaining quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.
- Graduate of an approved, accredited school of professional nursing required; BSN preferred.
- Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
- Current CPR certification required.
- Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred.
- Demonstrated excellent verbal and written communication skills required.
- Demonstrated strong interpersonal, organizational, and time management skills required.
- Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required.
- Current driver's license, a good driving record, and reliable transportation required.
