About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a Case Manager. In this role, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.

Responsibilities
  • Provide skilled nursing care and case management to clients and their families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of clients and their families.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support for clients and their families.
  • Communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination.
  • Document observations, assessments, and interventions in accordance with regulatory requirements and organizational policies.
Requirements
  • Graduate of an approved, accredited school of professional nursing required; BSN preferred.
  • Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts required.
  • Current CPR certification required.
  • Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred.
  • Demonstrated excellent verbal and written communication skills required.
  • Demonstrated strong interpersonal, organizational, and time management skills required.
  • Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required.
  • Current driver's license, a good driving record, and reliable transportation required.
