Job Title: Registered Nurse - Home Health

TotalMed Case Management Staffing is seeking a skilled and compassionate Registered Nurse to join our team for a travel nursing assignment in Amherst, New York.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in a home health setting. The ideal candidate will have a strong passion for delivering exceptional patient outcomes and a commitment to excellence in nursing practice.

Responsibilities:
  • Provide comprehensive nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address any identified concerns.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to care.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
Requirements:
  • Current RN licensure in the state of New York.
  • Minimum of 1 year of experience as a Registered Nurse in a home health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to delivering exceptional patient care.
  • Basic Life Support (BLS) certification.
