About the Role

VNA Health Group is seeking a dedicated and compassionate Registered Nurse to join our team as a Home Health Care Specialist. In this role, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and well-being.

Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including assessments, planning, and implementation of care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Develop and maintain strong relationships with patients, families, and caregivers to promote trust and understanding.
  • Stay up-to-date with industry developments and best practices through ongoing education and training.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Current RN licensure in the State of New Jersey.
  • At least 1 year of experience in home health care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on patient-centered care.
  • Basic computer skills and proficiency in electronic medical records.
