VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team in providing high-quality home health care services to patients in Essex County, New Jersey.
Job Summary:We are looking for a dedicated and experienced Registered Nurse to provide skilled nursing care to patients in their homes. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.
Responsibilities:- Assess and evaluate patient needs, developing individualized care plans to meet their unique requirements.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination of services.
- Document patient progress, updating care plans and outcomes as necessary.
- Identify teaching needs and develop strategies to educate patients and their families on health management and self-care.
- RN with Bachelor's degree in Nursing preferred.
- 1+ year of supervised clinical experience as a registered nurse.
- Excellent verbal and written communication skills.
- Strong computer skills.
- Excellent organizational and interpersonal skills.
- Valid driver's license and reliable transportation.
