VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Cedar Grove, New Jersey.
Job SummaryWe are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a member of our Home Health team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Assess and evaluate patient needs, developing individualized care plans to promote optimal health outcomes.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions.
- Document patient progress, updating care plans and outcomes as needed.
- Identify teaching needs and develop strategies to educate patients and their families on disease management and self-care.
- Active New Jersey RN License
- Bachelor's degree in Nursing preferred
- 1+ year of supervised clinical experience as a registered nurse
- Excellent verbal and written communication skills
- Strong computer skills
- Excellent organizational and interpersonal skills
- Car and valid driver's license
