VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Cedar Grove, New Jersey.
Job Summary:We are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a member of our Home Health team, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote optimal health and well-being.
Responsibilities:- Assess and evaluate patient needs, developing and implementing effective care plans to address physical, emotional, and social needs.
- Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between healthcare settings.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Document patient progress, outcomes, and care plans in accordance with regulatory requirements and VNA Health Group policies.
- Current RN licensure in New Jersey.
- 1+ year of supervised clinical experience as a Registered Nurse.
- Excellent verbal and written communication skills.
- Strong computer skills and proficiency in electronic medical records.
- Valid driver's license and reliable transportation.
