Job Title: Home Health RN

VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our team in providing high-quality home health care to patients in Essex County.

Job Summary:

The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Assess and evaluate patient needs, developing individualized care plans to ensure optimal health outcomes.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and families.
  • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
  • Document patient progress, updating care plans and outcomes as necessary.
  • Identify teaching needs and develop strategies to educate patients and families on disease management and self-care.
  • Monitor quality and appropriateness of services provided, ensuring compliance with regulatory requirements.
Requirements:
  • RN with Bachelor's degree in Nursing, preferred.
  • 1+ year of supervised clinical experience as a registered nurse.
  • Excellent verbal and written communication skills.
  • Strong computer skills.
  • Excellent organizational and interpersonal skills.
  • Valid driver's license and reliable transportation.
