Job Title: Behavioral Health Housing/Homeless Outreach Case Manager

Sea Mar Community Health Centers is seeking a skilled and compassionate Behavioral Health Housing/Homeless Outreach Case Manager to join our team. As a key member of our Behavioral Health program, you will play a vital role in providing comprehensive support services to individuals and families in need.

Job Summary:

The Behavioral Health Housing/Homeless Outreach Case Manager will work collaboratively with our mental health team to provide housing support interventions and referrals to community providers. This role requires a strong understanding of housing resources and a passion for empowering individuals to achieve stability and self-sufficiency.

Responsibilities:

  • Manage a caseload of clients and provide individualized support services to meet their unique needs.
  • Collaborate with the mental health team to design and implement housing support interventions.
  • Develop and maintain relationships with community providers to ensure seamless referrals and support services.
  • Provide education and advocacy to clients on housing resources and benefits.
  • Participate in team meetings and contribute to the development of program policies and procedures.

Requirements:

  • Bachelor's or Master's degree in Counseling, Social Work, Psychology, or related field.
  • Washington State Department of Health Agency Affiliated Counselor Certification.
  • Valid driver's license and access to a car.
  • Ability to pass a TB test and maintain current BLS CPR certification.
