Job Summary:

We are seeking a dedicated Care Manager to join our team in Dallas, TX. The successful candidate will work closely with members who require support with their mental and behavioral health needs.

Responsibilities:
  1. Evaluate member service needs and develop or contribute to development of care plans/service plans
  2. Provide education to members, their families and caregivers on services and benefits available to meet member needs
  3. Collaborate with resources, such as medical professionals and social services agencies, to provide comprehensive support
Requirements:
  • Master's degree in Mental Health or Social Work or Graduate from an Accredited School of Nursing
  • 2 – 4 years of related experience as a Licensed Behavioral Health Professional or RN with psychiatric experience
  • Proficiency in case management systems and strong clinical documentation skills
