About the RoleThis is an exceptional chance to join our team as a Senior Clinical Care Coordinator RN, utilizing your expertise in managing member cases within the scope of licensure. You will provide supervision and direction to non-RN clinicians, develop, monitor, evaluate, and revise care plans to meet member needs, and optimize healthcare across the care continuum.
In this role, you will perform face-to-face clinical assessments for identifying, evaluating, coordinating, and managing member needs, including physical health, behavioral health, social services, and long-term services and supports.
Responsibilities- Identify members at high risk for complications and coordinate care with the member and healthcare team.
- Manage members with chronic illnesses, co-morbidities, and disabilities to ensure cost-effective and efficient utilization of health benefits.
- Obtain thorough and accurate member histories to develop individualized care plans.
- Establish short- and long-term goals in collaboration with the member, caregivers, family, natural supports, physicians, and identify those who would benefit from alternative levels of care or other waiver programs.
You will have overall responsibility for developing care plans for services for the member and ensuring their access to these services. Additionally, you may assist in implementing member care plans by facilitating authorizations/referrals for service utilization, as appropriate, within benefits structures or through extra-contractual arrangements, as permissible.
RequirementsTo be successful in this role, you will require:
- An active, unrestricted RN license in applicable states.
- A minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role.
- May require state-specified certification based on state law and/or contract.

Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
