About the Role

Pinnacle Home Care is seeking a dedicated and compassionate Occupational Therapist to join our team of skilled home healthcare providers. As a key member of our team, you will play a vital role in delivering high-quality care to our patients in their homes.

Responsibilities
  • Evaluate patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  • Assess the home environment for hazards or barriers to independent living, identify equipment needs, and participate in the development of the total plan of care.
  • Develop treatment programs and establish goals for improved function, communicating plans of care to referring physicians and other healthcare professionals.
  • Teach new skills or retrain patients in daily activities lost due to illness or injury, in accordance with organizational policy.
  • Fabricate splints and instruct patients in the use of adaptive equipment to improve function.
  • Train patients in the use of prosthetic and/or orthotic devices.
  • Maintain accurate clinical records, clinical notes, and reports to physicians on changes in patients' conditions.
  • Maintain contact/communication with other personnel involved in patient care to promote coordinated and efficient care.
  • Attend and contribute to in-services, case conferences, and other meetings to ensure comprehensive plans of care for patients.
  • Identify patient and family/caregiver needs for other home health services and refer as appropriate.
  • Instruct patients' family/caregivers and other healthcare personnel in treatment regimes when indicated and appropriate.
  • Supervise certified occupational therapy assistants according to organizational policy and state regulations.
  • Participate in discharge planning for patients.
Requirements
  • Graduate of an Occupational Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.
  • Licensed to practice as an OT in the State of Florida.
  • Two years of clinical experience as an occupational therapist.
  • Current BLS Certification.
  • Demonstrates good verbal and written communication and organization skills.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • This position requires that the employee is willing and able to meet the physical requirements as necessary.
