Medical Care Coordinator RN

The Medical Care Coordinator RN plays a vital role in ensuring the delivery of high-quality care that fosters the well-being of each resident. This position involves collaborating with healthcare professionals to develop and implement individualized plans of care, monitoring resident conditions, and implementing necessary interventions.
Key Responsibilities
- Conduct regular rounds to assess resident conditions and document changes
- Ensure the implementation of care plans as directed by physicians and facility policies
- Administer medications as prescribed and perform narcotic reconciliation during shifts
- Collaborate with physicians to record visits in clinical records and identify significant changes in resident conditions
- Oversee and monitor certified nursing assistants to ensure quality care delivery
- Complete incident reports and discharge residents according to facility protocols
Requirements
This position requires an active Registered Nurse license in the state of Texas and completion of a RN program from an accredited institution. Preference is given to candidates with experience working in skilled nursing facilities.
Salary Range: $65,000 - $85,000 per year
