Job Summary:

Integrated Resources is seeking a skilled Licensed Practical Nurse to join our team. As a LPN, you will work under the direct supervision of a Registered Nurse or physician to provide high-quality patient care and treatment in a nursing home setting.

Responsibilities:

• Provide nursing care to patients on an assigned unit, utilizing your knowledge of illnesses, injuries, and diseases to make accurate evaluations and apply the nursing process.

• Collaborate with the care team to develop and implement individualized care plans, ensuring timely and comprehensive reevaluations and documentation as needed.

• Monitor patient outcomes and coordinate resources to meet post-institutional requirements, ensuring a smooth transition for patients and families.

Requirements:

• 2+ years of experience in a nursing home setting, with a focus on patient care and treatment.

• Ability to work in a fast-paced environment, with a high level of attention to detail and a commitment to delivering exceptional patient care.

• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Physical Requirements:

• Ability to stand for long periods of time, with frequent walking and bending.

• Ability to lift and move patients as needed, with a focus on patient safety and comfort.
