Position: Per Diem RN - Hospice Services

Lumina Hospice & Palliative Care is actively seeking a dedicated Per Diem Registered Nurse to become a vital part of our compassionate team. This role offers the unique opportunity to engage with programs and services that prioritize patient choice and dignity, ensuring comprehensive care for both patients and their families.

Key Responsibilities:

  • Conduct thorough assessments and ongoing evaluations of patient and family needs, effectively organizing resources to address those needs.
  • As a Per Diem RN, you will coordinate care across various hospice disciplines to ensure seamless service delivery.
  • Deliver nursing care that is attentive to the physical, emotional, and spiritual needs of patients.
  • Provide guidance to patients and families regarding symptom management, medication, and nutritional needs.
  • Support patients and families in making informed choices that align with their preferences by offering alternatives and encouragement.
  • Be present during end-of-life transitions, providing support to families and assisting with final arrangements.
  • Manage the admission of new patients, educate families, arrange necessary services and equipment, and procure medications as needed.
  • Collaborate effectively with other healthcare professionals, including physicians and care facilities, to ensure optimal patient care.
  • Maintain open communication with Lumina staff, functioning as an integral member of the patient care team.
  • Develop and update patient care plans in collaboration with interdisciplinary team members, ensuring accurate documentation and participation in team meetings.
  • Adhere to all Medicare regulations, providing timely and professional documentation of patient interactions and maintaining accurate patient records.

Qualifications:

  • Strong commitment to serving the needs of terminally ill patients.
  • Current, unencumbered Oregon Registered Nurse license.
  • Professional nursing experience in hospice, palliative care, home health, or equivalent settings.
  • Valid driver's license with a reliable vehicle for patient home visits.
  • Exceptional communication skills and proficiency in patient documentation and service coordination.
  • Proficient in using electronic tools for documentation and charting.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Physical capability to lift up to 40 pounds and assist patients as needed.
  • Bi-lingual skills, particularly in Spanish, are advantageous.
  • Willingness to share after-hours responsibilities with fellow nurses.

Schedule: This position is designed to provide coverage for open shifts as needed, with an expectation of flexibility and reliability. The average commitment is approximately two shifts per week, which may include days, evenings, or weekends.

*Candidates may be required to undergo a background check and drug screening upon receiving a job offer.
**New hires must complete tuberculosis screening and provide proof of hepatitis B vaccination or signed declination.
***Proof of COVID-19 vaccination is also required.
