Per Diem RN - Hospice Care

Lumina Hospice & Palliative Care is currently seeking a dedicated Per Diem Registered Nurse specializing in hospice care. This role provides a unique opportunity to engage with programs and services that prioritize patient choice and dignity, while delivering comprehensive care to patients and their families.

Key Responsibilities:

  • Conduct thorough assessments and evaluations of patient and family needs, coordinating resources to effectively address those needs.
  • As a Per Diem RN, you will oversee the care provided by various hospice disciplines.
  • Deliver nursing care that is attentive to the physical, emotional, and spiritual well-being of patients.
  • Guide patients and families in understanding symptom management, medication usage, and nutritional needs.
  • Support patients and families in making informed choices that align with their preferences, offering alternatives and assistance.
  • Provide compassionate support during end-of-life situations, helping families with final arrangements.
  • Facilitate the admission of new patients, educate families, coordinate services and equipment, obtain necessary physician orders, and manage medication procurement.
  • Collaborate with other healthcare professionals, including physicians and pharmacies, to ensure the highest quality of care for patients.
  • Maintain open and effective communication with Lumina staff, functioning as a vital member of the patient care team.
  • Develop and adjust patient care plans in collaboration with interdisciplinary team members, ensuring accurate documentation and participation in team meetings.
  • Ensure compliance with Medicare regulations through timely and professional documentation, maintaining up-to-date patient records and care plans.

Qualifications:

  • Strong commitment to serving the terminally ill.
  • Current, unrestricted Oregon RN license.
  • Professional nursing experience in hospice, palliative care, home health, or related fields.
  • Valid driver's license and reliable transportation for patient visits.
  • Excellent communication skills and experience in patient documentation and service coordination.
  • Proficient in using electronic tools for accurate and timely charting.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Physical capability to lift up to 40 pounds and assist with patient mobility as needed.
  • Spanish language proficiency is a plus.
  • Willingness to share after-hours responsibilities with other nursing staff.

This position is designed to offer flexibility, with an expectation of covering shifts as needed, typically averaging two shifts per week. Shifts may include days, evenings, or weekends, with on-call responsibilities as required.

Successful candidates will undergo a background check and drug screening following a job offer. New hires must complete tuberculosis screening and provide proof of hepatitis B vaccination or signed declination, as well as proof of COVID-19 vaccination.
