Per Diem RN - Hospice Services

Lumina Hospice & Palliative Care is seeking a dedicated Per Diem Registered Nurse specializing in hospice care. This role offers the chance to engage with programs that prioritize patient choice and dignity, delivering holistic care to patients and their families.

Key Responsibilities:

  • Conducts thorough assessments and ongoing evaluations of patient and family needs, coordinating resources effectively.
  • As a Per Diem RN, you will oversee the care provided by various hospice disciplines.
  • Delivers nursing care with a focus on the physical, emotional, and spiritual well-being of patients.
  • Provides guidance to patients and families regarding symptom management, medication administration, and nutritional support.
  • Facilitates informed decision-making for patients and families by presenting alternatives and offering support.
  • Attends to families during end-of-life situations, providing emotional support and assistance with arrangements.
  • Handles the admission of new patients, educating families, arranging necessary services, and procuring medications.
  • Collaborates with healthcare professionals, including physicians and pharmacies, to ensure comprehensive patient care.
  • Maintains open communication with Lumina staff, contributing as a vital member of the patient care team.
  • Develops and updates patient care plans in collaboration with interdisciplinary team members, ensuring accurate documentation.
  • Adheres to Medicare regulations, ensuring timely and professional documentation of all patient interactions and care plans.

Qualifications:

  • Strong commitment to serving terminally ill patients.
  • Current, unrestricted Oregon RN license.
  • Professional nursing experience in hospice, palliative care, or related fields is required.
  • Valid driver's license and reliable transportation for patient visits.
  • Excellent communication skills and experience in patient documentation and service coordination.
  • Proficient in using electronic tools and willing to learn medical software for accurate charting.
  • Ability to work independently and as part of an interdisciplinary team.
  • Physical capability to lift up to 40 pounds and assist patients as needed.
  • Bi-lingual skills, particularly in Spanish, are advantageous.
  • Willingness to share after-hours responsibilities with fellow nurses.

This position is designed to provide flexible coverage for open shifts, typically requiring availability for two shifts per week, which may include days, evenings, or weekends.
