Position Overview: The Multilingual Youth Support Coordinator for Residential and Foster Care Services is integral in delivering culturally sensitive case management aimed at the safe return and reunification of unaccompanied youth in care.

Key Responsibilities:

  • Collaborate closely with the Division of Unaccompanied Children Operations (DUCO) to manage intake and transition processes, ensuring legal representation is arranged when necessary.
  • Handle all intake and exit documentation, coordinate case planning and transportation, and provide translation services as required.
  • Act as a bridge between youth, their families, and their countries of origin, fostering connections that support potential reunification efforts.
  • Engage directly with youth on a weekly basis to evaluate placement needs and facilitate smooth transitions, meticulously documenting all interactions and service transitions.
  • Maintain an assigned caseload of unaccompanied youth in foster care, meeting regularly with the Program Supervisor to discuss concerns and progress towards reunification goals.
  • Plan for successful transitions of youth, involving family, sponsors, community resources, and other support systems.
  • Assist in the rotation of the emergency crisis phone and respond as necessary.

Documentation and Reporting:

  • Organize and submit all required reports across various databases, including intake and exit paperwork, assessment and service plans, and case management notes.
  • Attend agency or professional seminars, training, or conferences to enhance professional knowledge.
  • Adhere to all relevant codes, policies, and guidelines to ensure compliance.

Client Interaction:

  • Maintain direct contact with each assigned client, ensuring at least one documented, scheduled weekly meeting to discuss reunification options.
  • Collaborate with the Mental Health Therapist to develop culturally and developmentally appropriate treatment plans and outcome goals for each youth.
  • Support foster parents in their role as primary life skills educators.
  • Facilitate quarterly treatment service plans, goals, and individualized approaches for each youth on the caseload.
  • Develop community resources to support youth's treatment plans and discharge planning.
  • Establish and monitor aftercare treatment plans for all youth on the caseload.

Qualifications:

  • Bilingual in English/Spanish.
  • Bachelor's degree in behavioral sciences, human services, or related fields.
  • Preferred experience of 3 years in relevant fields, with a strong preference for child welfare and case management experience.
  • Completion of de-escalation and restraint training within the first 45 days of employment.
  • Demonstrated commitment to diversity, equity, and inclusion principles.
