Job Title:

Palliative Nurse Practitioner (NP) / Physician Assistant (PA)

Job Summary:

Elevance Health is seeking a highly skilled Palliative Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in Portland, ME. As a key member of our home-based palliative care team, you will provide comprehensive, collaborative medical care to patients with serious illnesses in the comfort of their own homes.

Responsibilities:
  • Collaborate with company physicians, patients' other physicians, and providers to develop complex plans of care
  • Provide services in home settings to patients with chronic and complex conditions
  • Develop and implement treatment plans based on patients' goals of care and current conditions
  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence, and crisis anticipation and prevention
  • Maintain contact with other clinical team members, patients' other physicians, and patients' other medical providers to coordinate optimal care and resources
  • Maintain patient medical records and medical documentation consistent with state regulations and company standards and policy
Requirements:
  • Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s)
  • DEA registration or eligibility required
  • CPR certification required
Preferred Skills:
  • 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care
  • Bedside hospital, oncology, emergency room (ER), or intensive care unit (ICU) experience highly preferred
  • Experience working with Electronic Medical Records (EMR) preferred
  • Bilingual in English/Spanish preferred
