Key Responsibilities:

As a Care Transition Specialist, you will play a critical role in implementing transition plans, communicating with treatment teams, patients, and families, and facilitating transportation arrangements. You will also be responsible for relaying important information to care managers, social workers, and insurance companies, and entering and executing transmissions of patient medical records.

Requirements:

To be successful in this role, you will need a high school education or GED, with 3 years of previous medical experience or 1 year with an advanced degree. Experience with community resources or insurance companies is desirable.
