Palliative Care Specialist

Elevance Health is seeking a skilled Palliative Nurse Practitioner to join our team in providing comprehensive, collaborative medical care to patients in the comfort of their own homes. As a Palliative Care Specialist, you will work closely with our team of healthcare professionals to develop complex plans of care that address the physical, emotional, and spiritual needs of our patients.
Key Responsibilities- Provide high-quality, patient-centered care to individuals with serious illnesses, focusing on relief from symptoms, pain, and stress.
- Collaborate with our interdisciplinary team, including physicians, nurses, and social workers, to develop and implement individualized care plans.
- Conduct thorough assessments, gather medical history, and perform physical exams to inform treatment decisions.
- Prescribe medications and therapies as needed, while ensuring safe and effective care.
- Maintain accurate and up-to-date patient records, adhering to regulatory requirements and company standards.
- Participate in ongoing education and training to stay current with best practices in palliative care.
- Current unrestricted Nurse Practitioner (NP) license or Physician Assistant (PA) license in applicable state(s).
- DEA registration or eligibility required.
- CPR certification required.
- 2 years of experience in home-based services, complex care cases, and hospice and/or palliative care preferred.
- Bedside hospital, oncology, emergency room (ER) or intensive care unit (ICU) experience highly preferred.
- Experience working with Electronic Medical Records (EMR) preferred.
- Bilingual in English/Spanish preferred.
