Palliative Care Nurse Practitioner Job Description

We are seeking a highly skilled Palliative Care Nurse Practitioner to join our team at Amedisys. As a Palliative Care Nurse Practitioner, you will play a critical role in delivering high-quality, patient-centered care to individuals with serious illnesses.

Responsibilities:
  • Provide palliative care services to patients in their homes, clinics, and other care settings.
  • Work collaboratively with physicians and other healthcare professionals to develop and implement individualized care plans.
  • Assess patients' physical, emotional, and spiritual needs, and provide appropriate interventions to address these needs.
  • Order and review diagnostic tests, lab results, and other medical information to inform treatment decisions.
  • Develop and implement treatment plans that address patients' medical, emotional, and spiritual needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient care and treatment plans accurately and thoroughly.
Qualifications:
  • Master's degree in Nursing or higher.
  • Current licensure as a Nurse Practitioner in the state where services will be provided.
  • Certification as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Experience working in palliative care or a related field.
  • Knowledge of disease-specific local coverage determinant (LCD) criteria for hospice eligibility and CMS Face-to-Face regulatory requirements.
