Job Summary

We are seeking a skilled RN Care Manager to join our Behavioral Health Community Partners program. This innovative initiative provides comprehensive care management services to eligible MassHealth enrollees with complex medical and mental health needs.

As a RN Care Manager, you will work closely with a multidisciplinary team to provide person-centered care to patients with a focus on addressing social determinants of health and systemic inequities. You will be responsible for collaborating with care coordinators, clinicians, and other stakeholders to ensure that patients receive necessary services and have access to resources that support their health and well-being.

This is an excellent opportunity for a nurse with a passion for working with vulnerable populations to make a meaningful difference in the lives of our patients.

Responsibilities
  • Provide clinical assistance to care coordinators with panels of approximately 45 clients each, including chronic disease management and medication support.
  • Collaborate with integrated care team members to ensure patients' needs are addressed and treatment barriers are reduced.
  • Engage vulnerable populations as part of a multidisciplinary outreach team, including home visits, outreach to hospitals, homeless shelters, and other settings as needed.
  • Perform timely follow-up and medication support and education following transitions of care.
  • Connect patients to various needed supportive services and resources.
  • Provide clinical oversight and tracking for annual comprehensive health assessments.
  • Participate in developing patient-centered care plans for patients on their panel.
Qualifications
  • Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
  • Nursing experience with complex patients preferred.
  • BSN preferred but not required.
  • Experience working with vulnerable populations, including persons with a history of trauma, those experiencing socioeconomic stress, homelessness, or substance use disorders.
  • Experience working on cross-disciplinary, integrated teams.
  • Strong interpersonal skills and clinical problem-solving.
  • Comfort with working in an autonomous clinical and outreach setting.
  • Capability working with data to track and measure performance.
