Job Summary:
We are seeking a skilled Nurse Care Manager to join our Behavioral Health Community Partners program. This innovative initiative provides support to eligible MassHealth enrollees with complex medical and mental illnesses, focusing on individuals lacking stable housing. As a Nurse Care Manager, you will work closely with care coordinators and patients to provide person-centered care, using a population health lens to deliver preventative and reactionary services.
Responsibilities:
- Collaborate with care coordinators and patients to develop and implement patient-centered care plans.
- Provide clinical assistance to care coordinators, including chronic disease management and medication support.
- Engage vulnerable populations through home visits, outreach to hospitals, and other settings as needed.
- Connect patients to supportive services and resources, including substance use treatment and housing assistance.
- Participate in quality improvement efforts and contribute to the development of patient-centered care plans.
Qualifications:
- Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
- Nursing experience with complex patients preferred.
- Strong interpersonal skills, clinical problem-solving, and ability to work in an autonomous setting.
- Comfort with working with data to track and measure performance.
- Bilingual skills in Spanish and English strongly preferred.
