Job Title: Clinical Care Manager RN Cambridge Area

We are seeking a highly skilled Clinical Care Manager RN to join our team at Commonwealth Care Alliance. As a Clinical Care Manager RN, you will be responsible for providing intensive monitoring, follow-up, and clinical care management to a panel of health plan members with complex medical, behavioral, and social care needs.

Key Responsibilities:
  • Develop and implement comprehensive care plans for assigned members
  • Provide education and support to members to help them navigate the healthcare system
  • Collaborate with interdisciplinary care teams to ensure seamless care coordination
  • Conduct routine and post-discharge assessments to identify and address member needs
  • Monitor and manage member data to inform proactive outreach and clinically appropriate assessments
Requirements:
  • Active RN license in good standing in the state of assignment
  • Minimum 3 years of clinical and care management experience, including experience caring for individuals with complex medical, behavioral, and social needs
  • Excellent critical thinking, organizational, time management, and problem-solving skills
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
Preferred Qualifications:
  • Experience working in a health plan, with Medicare Advantage and/or dually eligible population
  • Experience with a care management platform, such as Guiding Care
  • Experience with substance use disorders
