Key Responsibilities- Develop and implement care plans that address the unique needs of our patients, including outreach and engagement to formally enroll referred children into our care management program.
- Conduct thorough assessments and evaluations to identify areas of need and develop strategies to address them, ensuring that all necessary supports are in place.
- Collaborate with healthcare providers and other stakeholders to establish and maintain effective communication plans, ensuring that all team members are informed and aligned.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Provide education and support to patients and their families, empowering them to take an active role in their care.
- Advocate for additional services and linkages as necessary, ensuring that our patients receive comprehensive and coordinated care.
- Maintain accurate and up-to-date documentation of care management activities, ensuring compliance with health IT system requirements.
- Bachelor's degree in human services or a related field from an accredited institution.
- At least two years of experience in human services, with a focus on mental health and substance abuse.
