Job Title: LPN Home Health Weekend

We are seeking a skilled and compassionate LPN to join our home health team on a weekend schedule. As a LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Provide direct and indirect nursing care to patients in their homes, under the supervision of an RN.
  • Assess patient needs and develop individualized care plans to meet their physical, psychological, and social needs.
  • Administer medications and treatments as ordered by physicians, and document patient responses and outcomes.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Perform routine patient assessments, including vital signs, patient weights, and specimen collection.
  • Provide education and support to patients and families on disease management, medication adherence, and self-care techniques.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements:
  • Graduate of an LPN school.
  • Current unencumbered licensure with the WV Board of Practical Nursing, or appropriate state board where services will be provided, as a practical nurse.
  • Must possess valid WV or neighboring state driver's license and must maintain WV or neighboring state minimum auto insurance.
  • CPR certification within 30 days of hire date.
Preferred Qualifications:
  • One year of Home Care Nursing experience.
Work Environment:

The work environment for this position will be primarily in patients' homes, with occasional travel to our office or other healthcare facilities. You will be exposed to various environments, including homes with pets, children, and other family members. You will also be exposed to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.

As a LPN, you will be required to work independently and make sound, independent judgments based on scientific principles. You will also be expected to collaborate with other healthcare professionals to ensure the best possible outcomes for our patients.
