Job Title: LPN: Weekend Premium

We are seeking a skilled and compassionate LPN to join our team as a Weekend Premium LPN. As a Weekend Premium LPN, you will provide high-quality patient care to our clients in their homes, working closely with our healthcare team to ensure the best possible outcomes.

Responsibilities:
  • Provide direct and indirect nursing care to patients in their homes, under the supervision of an RN.
  • Assess patient needs and develop individualized care plans to meet their physical, psychological, and social needs.
  • Implement and evaluate patient care according to established policies and procedures.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient care and progress in a timely and accurate manner.
  • Perform various nursing tasks, including vital signs, patient weights, specimen collection, and wound care.
  • Administer medications as ordered by physicians and authorized by RNs.
  • Collaborate with the healthcare team to identify and address patient needs, and to develop and implement plans to meet those needs.
Requirements:
  • Graduate of an LPN school.
  • Current unencumbered licensure with the WV Board of Practical Nursing, or appropriate state board where services will be provided, as a practical nurse.
  • Current CPR card.
  • Valid WV or neighboring state driver's license and minimum auto insurance.
  • One year of Home Care Nursing experience preferred.
Preferred Qualifications:
  • One year of Home Care Nursing experience.
Work Environment:

The work environment for this position will be in the patient's home, which may require extensive travel. The LPN will be exposed to various environments, including homes with different layouts and conditions. The LPN must be able to adapt to these environments and provide high-quality care in a variety of settings.

Skills and Abilities:
  • Ability to perform concentrated and complex mental activity with frequent involvement in complex and highly technical situations.
  • Ability to work successfully under highly stressful conditions and adapt to varying workloads and work assignments on a constant basis.
  • Ability to comprehend and perform oral and written instructions and procedures.
  • Strong communication skills, written and verbal.
  • Math skills to enable calculation of drug dosages and infusion rates.
  • Demonstrates knowledge of and adherence to patient rights.
  • Ability to work weekend, holidays, and all shifts.
  • Demonstrates knowledge of and compliance with safety policies and procedures.
  • Demonstrates knowledge of compliance of Federal, State, and local regulations/laws.

This is a non-exempt position with a scheduled weekly hours of 40. The shift is Weekend (United States of America). The company is SHC WVUHS Home Care, and the cost center is 369 SHC Home Health Clarksburg B. The address is 2673 Davisson Run Rd, Clarksburg, West Virginia.
