Job Title: LPN Home Health Weekend

We are seeking a skilled and compassionate LPN to join our team as a Home Health Weekend Care Specialist. As a key member of our care team, you will provide high-quality nursing care to patients in their homes, ensuring their comfort, safety, and well-being.

Responsibilities:
  • Provide direct and indirect nursing care to patients in their homes, under the supervision of an RN.
  • Implement and evaluate patient care plans, in collaboration with the patient, family, and healthcare team.
  • Conduct continuing assessments, identify patient needs, and document care plans in the patient's chart.
  • Administer medications, perform treatments, and provide patient education as ordered by the physician.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in multidisciplinary team meetings to discuss patient care and develop care plans.
Requirements:
  • Graduate of an LPN school.
  • Current unencumbered licensure with the WV Board of Practical Nursing or appropriate state board.
  • Valid WV or neighboring state driver's license and minimum auto insurance.
  • CPR certification within 30 days of hire date.
Preferred Qualifications:
  • One year of Home Care Nursing experience.
Work Environment:

As a Home Health Weekend Care Specialist, you will work in patients' homes, which may require extensive travel. You will be exposed to various environments, including homes with different conditions, and may encounter unpredictable situations.
