Job Title: LPN Home Health

We are seeking a skilled and compassionate LPN to join our team at WVUHS Home Care, LLC. As a LPN Home Health, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Job Summary:

The LPN Home Health will be responsible for providing direct and indirect nursing care to patients in their homes, under the supervision of an RN. This includes assessing patient needs, developing and implementing care plans, and evaluating patient outcomes.

Responsibilities:
  • Provide basic patient care according to policies and procedures
  • Use the nursing process to meet patient physical and psychological needs
  • Perform and monitor patient teaching according to policy and procedures
  • Coordinate care to ensure medications are administered as ordered by a physician
  • Communicate patient care problems or potential problems to the primary RN and Administrative staff
  • Maintain patient confidentiality and adhere to patient rights
Requirements:
  • Graduate of an LPN school
  • Current unencumbered licensure with the WV Board of Practical Nursing, or appropriate state board where services will be provided
  • Valid WV or neighboring state driver's license and minimum auto insurance
  • CPR card
Preferred Qualifications:
  • One year of Home Care Nursing experience
Work Environment:

The LPN Home Health will work in a variety of home settings, including private residences and assisted living facilities. The work environment may be fast-paced and unpredictable, with exposure to bodily fluids, human tissues, and contagious diseases.

Skills and Abilities:
  • Ability to perform concentrated and complex mental activity
  • Ability to work successfully under highly stressful conditions
  • Ability to make sound, independent judgments based on scientific principles
  • Strong communication and math skills
Additional Information:

The LPN Home Health will be required to work weekend, holidays, and all shifts. The position is non-exempt and is subject to the company's policies and procedures.
