Job Title: LPN Home Health

We are seeking a skilled and compassionate LPN to join our team of healthcare professionals in providing high-quality home health care services to our patients. As an LPN Home Health, you will be responsible for delivering patient-centered care in the comfort of their own homes, promoting independence, and improving their overall quality of life.

Key Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to meet their unique requirements.
  • Administer medications, perform wound care, and provide other treatments as prescribed by physicians, ensuring accurate and timely delivery of care.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
  • Document patient information, progress, and care provided, maintaining accurate and up-to-date records.
Requirements:
  • Current licensure as a Licensed Practical Nurse (LPN) in the state of West Virginia or neighboring states.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time effectively.
  • Basic computer skills and proficiency in electronic medical records (EMRs).
Preferred Qualifications:
  • One year of experience in home health care or a related field.
  • Certification in a specialty area, such as wound care or infusion therapy.
Work Environment:

As an LPN Home Health, you will work in patients' homes, providing care in a variety of settings, including private residences, assisted living facilities, and nursing homes. You will be required to travel to different locations, using your own vehicle, and may be exposed to various environmental conditions, such as inclement weather.
