We are seeking a highly skilled Registered Nurse Case Manager to join our team at the Texas Health & Human Services Commission. As a Registered Nurse Case Manager, you will be responsible for providing complex nursing care to individuals residing at our State Supported Living Centers. You will work closely with our interdisciplinary team to ensure that individuals receive high-quality, holistic care that meets their unique needs.
Key Responsibilities- Conduct comprehensive physical assessments and develop individualized care plans to ensure optimal health outcomes.
- Provide hands-on nursing care, including guidance for LVNs and direct care staff to implement health care plans.
- Collaborate with the interdisciplinary team to ensure seamless communication and coordination of care.
- Monitor and document health status, identifying emerging issues and taking proactive steps to address them.
- Participate in quality assurance reviews and contribute to the development of nursing policies and procedures.
- Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.
- At least one year of experience as a practicing registered nurse, with a preference for experience in assessment and documentation of health status.
- Strong knowledge of nursing techniques, standards, and procedures, as well as health care laws and regulations.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals with intellectual and developmental disabilities, their families, and staff members.
