Key Responsibilities:
  • Collaborate with healthcare teams to develop and implement effective care plans
  • Conduct thorough assessments of patient needs and develop strategies to address them
  • Coordinate with social services to ensure seamless transitions of care
  • Monitor and analyze patient data to identify trends and areas for improvement
  • Develop and maintain relationships with community resources to support patient needs
Requirements:
  • Graduation from an accredited School of Nursing
  • Minimum of 1 year of acute care experience
  • Valid Texas RN License/Eligible Compact License
  • Current BLS certification from American Heart Association
  • BSN required or must be obtained within 3 years of hire
  • ACM certification preferred
  • Previous Case Management experience preferred
  • Previous leadership/management experience preferred
  • Proficient typing skills
  • Conflict management skills
  • Ability to trouble shoot/problem-solve
  • Ability to multi-task with multiple computer systems
