Work ScheduleThis is a full-time position with a schedule of Monday through Friday, from 8 AM to 5 PM.
Role ResponsibilitiesThe Case Management team operates primarily within inpatient units across the UMC Health System, including the Emergency Center. Collaborating closely with Social Services, our focus is on Utilization Review, ensuring effective care coordination and facilitating discharge planning for our patients.
The Case Manager plays a vital role in delivering high-quality care through optimal resource allocation, enhancing the efficiency of hospital services, and contributing to the financial stability of the healthcare system.
Qualifications- Graduation from an accredited School of Nursing
- A minimum of 1 year of experience in acute care settings
- Possession of a valid Texas RN License or an eligible Compact License
- Current BLS certification from the American Heart Association
- BSN required or must be obtained within 3 years of employment
- ACM certification is preferred
- Previous experience in Case Management is advantageous
- Leadership or management experience is a plus
- Proficient typing skills are essential
- Strong conflict management abilities are required
- Problem-solving and troubleshooting skills are necessary
- Ability to manage multiple tasks across various computer systems is crucial
