Job Summary

The Epilepsy Florida team is seeking a skilled Transition Coordinator to facilitate the smooth transition of patients from pediatric to adult healthcare settings. This role involves monitoring, tracking, and reporting on the transition process, addressing barriers, and maintaining accurate documentation in the Electronic Health Record (EHR). The ideal candidate will have a strong understanding of healthcare systems, including pediatric and adult healthcare settings, medical terminology, and healthcare processes.

Key Responsibilities
  • Monitor and track patient transitions, identifying and addressing barriers to care.
  • Maintain accurate documentation in the EHR, ensuring seamless communication between healthcare providers.
  • Collaborate with healthcare providers, community resources, and patients to ensure a smooth transition.
  • Develop and implement strategies to improve transition outcomes, including patient education and support.
  • Participate in program development and evaluation, contributing to the growth and improvement of Epilepsy Florida's services.
Requirements
  • Bachelor's degree in a healthcare-related field and/or 2+ years' experience in care coordination, case management, or a related field within a healthcare setting.
  • Strong understanding of healthcare systems, including pediatric and adult healthcare settings, medical terminology, and healthcare processes.
  • Excellent communication and interpersonal skills, with the ability to collaborate effectively with patients, families, healthcare providers, and community resources.
  • Empathy and a commitment to patient well-being are essential in this role.
