KEY RESPONSIBILITIES:
- Act as a liaison among various stakeholders
- Maintain accurate documentation and records
- Evaluate eligibility and support requirements
- Monitor client satisfaction and service delivery
- Facilitate referrals to community resources
- Develop tailored service plans for individuals
- Provide education and training in the human services sector
- Advocate for necessary services on behalf of clients
ADDITIONAL DUTIES:
- Conduct ongoing training sessions
- Execute tasks as assigned by supervisory personnel
REQUIRED QUALIFICATIONS:
- Strong verbal and written communication abilities
- Understanding of developmental and intellectual disabilities
- Knowledge of treatment methods and intervention strategies
- Experience in creating and executing individualized service plans
- Ability to coordinate with service providers and work collaboratively
- Demonstrated respect for clients and their families
EDUCATION AND EXPERIENCE:
A Bachelor's degree in a human services discipline is mandatory. Experience working with individuals with developmental and intellectual disabilities is preferred. A valid Driver's License is also required.
