Job Overview:

The Intensive Case Management (ICM) Caseworker is responsible for addressing obstacles to safety and well-being for particularly vulnerable refugee populations, including asylees and other individuals eligible for ORR services.

This role involves providing strengths-based, client-focused case management to various groups, including single-headed refugee households, survivors of domestic and gender-based violence, older refugees, youth and young adults lacking parental or guardian support, individuals facing mental health challenges, those with physical disabilities or medical issues, newly arrived refugees, and members of the LGBTQ community.

The ICM Caseworker collaborates closely with clients, colleagues, and external organizations to create tailored service plans that align with individual client needs and preferences, utilizing a comprehensive array of financial, medical, social, vocational, and other resources in accordance with eligibility and program guidelines.


Major Responsibilities:

Key Duties Include:


Assess client eligibility for ICM services and conduct intake evaluations, explaining program expectations, outcomes, and confidentiality to each prospective client.

Formulate personalized, SMART self-sufficiency plans for each participant in the ICM program.


Review client progress biannually during active program participation to assess goal attainment and collaborate with clients to identify emerging needs and necessary actions.


Guide clients through healthcare and social service systems, which may involve assisting with benefit applications and coordinating appointments.

Engage in community outreach and maintain a robust network of service providers to meet client requirements.

The ICM Caseworker is also expected to foster strong relationships with local and state refugee coordinators and health officials to ensure comprehensive client support.


Provide clients with in-depth orientation on various topics, including women's health, transportation systems, and local healthcare navigation.

Address client crises, seeking supervisor guidance as necessary.


Assist clients in applying for financial aid when applicable and maintain accurate records of financial assistance in case files.


Ensure timely and accurate documentation in client case files to reflect all services provided throughout the client's engagement; record all services in the organization's database, Efforts to Outcomes (ETO).

Participate in relevant training sessions and meetings at both local and national levels.


May mentor and oversee the activities of volunteers and interns.


Perform other duties as assigned.


Job Requirements:

Education:
A bachelor's degree in social work, public health, or a related field is preferred.

Work Experience:
A minimum of 1-2 years of case management experience with similar populations and/or client needs is required.

Demonstrated Skills & Competencies:
Exceptional relationship-building, diplomacy, and networking abilities; capacity to effectively cultivate internal and external partnerships.

Proven success in working and communicating effectively within a multicultural environment.

Self-motivated with strong problem-solving skills and the ability to prioritize and manage time efficiently.

Attention to detail and accuracy in all work outputs.

Fluency in English is required, both spoken and written; proficiency in languages spoken by client groups is preferred, including Dari/Pashto.


Proficient in Microsoft Office applications (Word, Excel, Outlook); capable of utilizing the internet and other digital tools for data entry, research, and problem-solving tasks.

Possession of a valid driver's license and access to a personal, insured vehicle is necessary.

Working Environment:


The role encompasses a blend of standard office settings, remote work, and field activities within the service delivery area to fulfill the outlined responsibilities.

Occasional weekend and/or evening work may be required.
