Case Manager

Centers for Independence is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in ensuring that our customers have access to the program services they need to achieve their goals.
Job Summary:The Case Manager will be responsible for developing and implementing case management plans that meet the unique needs of our customers. This will involve working closely with customers to understand their goals and objectives, and then developing a plan to help them achieve those goals.
Key Responsibilities:- Develop and implement case management plans that meet the unique needs of customers
- Work closely with customers to understand their goals and objectives
- Assist customers in accessing program services and benefits
- Monitor customer progress and make adjustments to case management plans as needed
- Collaborate with other team members to ensure that customers receive comprehensive support
- High school diploma or equivalent required
- Associate's degree from an accredited college or university preferred
- Proficient in Microsoft Office products
- Excellent organizational, written, and verbal communication skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Current Non-Violent Crisis Intervention certification or completed within first 90 days of hire
