Hospice Case Manager Job Description
We are seeking a skilled Hospice Case Manager to join our team at Amergis. As a Hospice Case Manager, you will be responsible for providing high-quality patient care and managing the care plan for patients in their homes and facilities.
Key Responsibilities:
- Provide patient care and manage the care plan in accordance with hospice policies and procedures.
- Conduct regular assessments and evaluations to ensure the patient's needs are met.
- Collaborate with the interdisciplinary team to develop and implement the patient's care plan.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Manage and coordinate the patient's care plan, including scheduling appointments and arranging for necessary services.
Requirements:
- Current RN licensure and certification in hospice care.
- Previous experience in hospice care or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
