Job Title: Patient Care Specialist
We are seeking a dedicated and compassionate Patient Care Specialist to join our team at Fresenius Medical Care. As a key member of our hemodialysis care team, you will play a vital role in delivering exceptional patient care and ensuring the highest quality treatment outcomes.
About the Role:
- Deliver comprehensive patient care services, including assessment, planning, intervention, and evaluation.
- Collaborate with interdisciplinary teams to develop and implement individualized treatment plans.
- Provide education and support to patients and their families to ensure seamless transitions and optimal care.
- Work closely with healthcare professionals to coordinate care and address patient needs.
- Contribute to quality improvement initiatives and participate in staff training and development programs.
Requirements:
- Current RN licensure in good standing.
- Minimum 9 months of experience as a Registered Nurse.
- Chronic/acute hemodialysis experience preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
Salary: $43.00 - $54.50 per hour
