  • Home Visits: Conduct visits to patients' homes as outlined in the care plan or as directed by leadership.
  • Professional Development: Attend required training and educational programs to enhance skills and knowledge in the field.

Qualifications

  • Licensure: A valid driver's license or NYS Non-Driver photo ID card is required based on operational needs.
  • Education: An Associate's Degree in Social Work, Psychology, or a related field, or equivalent experience in a social agency is necessary.
  • Experience: At least one year of experience in social work within a mental health, healthcare, or human services environment is required. Bilingual skills may be necessary based on operational needs.
