Job Title: Director of Inpatient Care Management

Alameda Health System is seeking a highly skilled and experienced Director of Inpatient Care Management to lead our care management operations across all facilities. As a key member of our leadership team, you will be responsible for ensuring efficient patient flow, from admission to discharge, and promoting high-quality patient care.

Key Responsibilities:
  • Develop and implement care management strategies to optimize patient outcomes and reduce healthcare costs.
  • Lead a team of care management professionals to ensure seamless coordination of patient care across all facilities.
  • Collaborate with physicians, nurses, and other stakeholders to develop and implement care plans that meet the unique needs of each patient.
  • Monitor and analyze care management metrics to identify areas for improvement and develop strategies to address them.
  • Develop and maintain relationships with payers, vendors, and other external partners to ensure optimal reimbursement and resource utilization.
  • Stay up-to-date with regulatory requirements and industry best practices to ensure compliance and excellence in care management.
Requirements:
  • Master's degree in Nursing or Social Work or related field.
  • Seven to ten years of experience in case management or quality improvement, with progressive management experience in a large healthcare organization.
  • Active licensure as a Registered Nurse in the State of California or valid license as a Clinical Social Worker issued by the State of California Board of Behavior Science Examiners.
  • Basic Life Support Certification issued by the American Heart Association and Crisis Prevention Intervention Training.
Preferred Qualifications:
  • Certification in Case Management, CCMC or ACM.
