About Your WorkAs an Inpatient Care Manager RN, you'll play a critical role in our team's success. Your responsibilities will include initial and concurrent utilization review, discharge planning, coordination of care, and referrals for hospitalized members.
You'll work closely with physicians, the care team, and our Medical Directors to ensure that patients receive the right care at the right time. Your role will involve assessing, coordinating, and implementing care plans to ensure optimal experience, outcomes, and cost.
You'll also establish relationships and foster respect and rapport with our Health Management Operations and Health Value Teams. We're looking for someone with a strong commitment to patient-centered care and a passion for promoting health, wellness, and patient experience.
Your FocusAs an Inpatient Care Manager RN, your focus will be on:
- Conducting initial and concurrent review and discharge planning/care coordination, utilizing evidence-based criteria and collaboration with our Medical Directors.
- Monitoring and continually assessing and evaluating member severity of illness, intensity of services, and response to treatment.
- Applying clinical knowledge, critical thinking skills, and solutions-oriented problem-solving and strong decision-making approaches.
We're looking for someone with:
- Working knowledge and application of utilization management standards and medical necessity criteria.
- Effective and meaningful communication skills with both internal and external customers.
- Development of collaborative relationships and work with our Providers and fostering a shared vision and values.
- Bachelor of Science in Nursing (BSN) or equivalent experience.
- Current unrestricted California RN licensure.
- Minimum of 3-5 years recent acute/hospital clinical experience.
- Computer skills proficient, experience in EMR, Microsoft Office Suite.
