Responsibilities

  • Conducting thorough nutrition assessments at enrollment and semi-annually, adhering to CMS guidelines;
  • Evaluating participants' psychosocial, functional, and behavioral factors related to food access, selection, preparation, physical activity, and health condition understanding;
  • Collaborating with other IDT members or healthcare professionals as needed;
  • Developing individualized interventions focused on the etiology of the problem;

To ensure quality care, our Dietary Specialist must possess excellent analytical skills, be able to problem-solve, and provide viable interventions.
