This role is crucial in identifying malnutrition, morbid obesity, food insecurity, and developing individualized goals for participants with complex medical conditions.

  • A thorough nutrition assessment is conducted at enrollment and semi-annually to determine nutritional status and identify potential risks.
  • The dietitian utilizes screening tools, medical history, laboratory data, and diet history to assess nutritional risk factors.

Nutrition Diagnosis and Planning

  • The dietitian identifies patterns and possible causes for the nutrition problem, documenting the diagnostic statement in PES (problem, etiology, and signs/symptoms) format.
  • An individualized nutrition plan is formulated, focusing on the etiology of the problem, nutrient requirements, knowledge, and abilities are assessed, and personalized nutrition education and counseling are provided.

Care Coordination and Documentation

  • The RD completes relevant, accurate, and timely documentation in the participant's electronic medical record, ensuring seamless coordination with the interdisciplinary team.

The estimated salary for this position is $72,000 - $92,000 per year, depending on experience and qualifications.
