MDS Coordinator Job Summary

The MDS Coordinator RN is responsible for coordinating and assisting with the completion and submission of accurate and timely interdisciplinary MDS Assessments, CCAs, and Care Plans according to CMS RAI Manual Regulations.

Key Responsibilities
  • Coordinate and assist with the completion and submission of MDS Assessments, CCAs, and Care Plans
  • Report changes in patient condition to the DON
  • Provide education to direct care associates on updates to CMS RAI Manual or Skilled Nursing Facility Regulations
  • Assist with review of Interdisciplinary Comprehensive Care Plan
  • Review Final Validation Reports and attest to completion and acceptance of assessments into CMS QIES system
  • Review CMS Reports to identify assessments completed or submitted late and develop systems to prevent reoccurrence
  • Attend and participate in Daily PPS Meeting, Monthly Triple Check, and other meetings as requested
Requirements
  • Associate's or bachelor's degree in nursing from an accredited college or university
  • Currently licensed/registered in applicable State
  • Two (2) years' nursing experience, with geriatric nursing experience preferred
  • CRN C Certification (clinical compliance)
  • CPR certification upon hire or obtain during orientation
