Clinical Case Manager

We are seeking a compassionate and adaptable In-Home Clinical Case Manager to join our Family Preservation Team. As a Permanency Therapist, you will provide clinical interventions and case management to children and families through a trauma-informed, strength-based approach.
Key Responsibilities:
- Conduct family and individual clinical work with adults, children, foster families, and birth families
- Develop treatment plans to strengthen families and help them achieve safety, permanency, and well-being
- Collaborate with multiple systems, including homes, schools, child protection, and courts
Requirements:
- Master's Degree in social work, counseling, or marriage & family therapy with at least 2 years of direct work experience
- Ability to train in or experience with evidence-based practices
- Computer skills and experience with Microsoft Outlook, Word, Excel
