Job Summary:

As a CRSW Case Manager at Catholic Medical Center, you will play a vital role in supporting patients on their journey to sustained recovery from substance use disorders. You will engage with patients, conduct intake and follow-up interviews, and collaborate with the Doorway team to plan strategies for communication, referrals, and transitions of care. You will also assist patients in accessing healthcare services and community resources, supporting them in achieving their recovery goals.

Key Responsibilities:

  • Engage with patients referred to The Doorway program and conduct intake, follow-up, and discharge interviews to assess patient needs and enroll them in the program.
  • Collaborate with The Doorway team to plan strategies for communication, referrals, and transitions of care.
  • Assist patients in accessing healthcare services and community resources, supporting them in achieving their recovery goals.
  • Document all patient interactions accurately and confidentially in electronic medical records.
  • Participate in team meetings and educational initiatives to promote overdose awareness and naloxone administration in the community.
Requirements:

  • High school diploma required - Recovery Support Worker Certification must be obtained within 9 months of start date.
  • Exceptional interpersonal skills and the ability to work effectively with diverse groups.
  • Critical thinking abilities and a proactive approach to problem-solving.
  • Strong verbal communication skills to convey important instructions and information.
  • Willingness to work in a fast-paced environment with frequent interruptions and adapt to unexpected situations.
  • Basic computer skills and the ability to learn new software systems.
  • Experience or willingness to work with vulnerable populations, and understanding of mental illness and substance use disorders.
